Why Rusholme sees more B12 deficiency than average
Two specific clinical patterns make B12 deficiency particularly common in our Rusholme patient population:
- Long-term metformin use. Type 2 diabetes is highly prevalent in the South Asian community, and many Rusholme patients have been on metformin for 5–15 years. Metformin interferes with calcium-dependent B12 absorption in the terminal ileum, and the longer the duration, the higher the deficiency rate. British Society for Haematology guidelines recommend periodic B12 screening in long-term metformin users.
- Long-term PPI use. Omeprazole, lansoprazole and esomeprazole reduce gastric acid, which is required to release B12 from food proteins. Long-term PPI use (often for reflux, gastritis or Helicobacter eradication) is another well-documented cause.
We also see dietary deficiency in long-term unsupplemented vegetarians, post-bariatric malabsorption, and a smaller cohort of pernicious anaemia.
The BNF protocol
New B12 deficiency without neurological involvement: six 1mg hydroxocobalamin injections over 2 weeks, alternate days. With neurological symptoms (peripheral neuropathy, glossitis, ataxia, cognitive change): alternate-day injections until improvement plateaus, then 2-monthly maintenance. Pernicious anaemia: 3-monthly lifelong maintenance. Drug-induced deficiency: typically the deficiency persists for as long as the metformin or PPI continues; some patients can come off PPIs once gastritis settles, others remain on lifelong replacement.
