With neurological symptoms (peripheral neuropathy, glossitis, ataxia, cognitive change): alternate-day injections until improvement plateaus, then 2-monthly maintenance. Pernicious anaemia: 3-monthly lifelong maintenance. Dietary deficiency: 3-monthly until corrected, then often oral cyanocobalamin 1mg daily long-term.
