South Asian BMI thresholds — why 2.5 points matters

Per NICE CG189, the BMI thresholds for considering weight-management treatment are reduced by 2.5 points for South Asian, Chinese, Middle Eastern, Black African and African-Caribbean adults: 27.5 instead of 30 for the unconditional overweight band, and 25 instead of 27.5 with a weight-related comorbidity. The reason: South Asian adults at any given BMI have a higher proportion of visceral fat and a higher risk of type 2 diabetes, dyslipidaemia and ischaemic heart disease than white European adults at the same BMI. This is reflected in our GLP-1 eligibility decisions for Rusholme patients — we don't apply a single one-size threshold.

Mounjaro and Wegovy — the clinical programme

We prescribe Mounjaro (tirzepatide, dual GLP-1/GIP, NICE TA1026) and Wegovy (semaglutide 2.4mg, NICE TA875). SURMOUNT-1 showed ~21% weight loss on Mounjaro at 72 weeks; STEP-1 showed ~15% on Wegovy at 68 weeks. Mounjaro escalates 2.5 → 5 → 7.5 → 10 → 12.5 → 15mg weekly; Wegovy 0.25 → 0.5 → 1 → 1.7 → 2.4mg weekly. We follow MHRA Drug Safety Updates. For South Asian patients with established prediabetes (HbA1c 42–47 mmol/mol) or type 2 diabetes (HbA1c ≥48 mmol/mol per NICE CKS Diabetes), Mounjaro often offers the additional HbA1c reduction benefit on top of weight loss.

Baseline bloods and the metformin/B12 cross-reference

Every Rusholme patient is offered a baseline panel before any first prescription — HbA1c, U&E, LFTs, TSH/T3/T4, lipid profile, vitamin D — via our Rusholme blood tests service, with 24–48-hour turnaround from a UKAS-accredited lab. A large proportion of our Rusholme patients are already on metformin from their GP, and long-term metformin (5+ years) is a recognised cause of B12 deficiency through interference with calcium-dependent absorption in the terminal ileum. We screen B12, ferritin and folate as a matter of course; if low we can move straight to our Rusholme B12 injections page for loading-dose hydroxocobalamin.

Ramadan and dose adjustment

For patients fasting during Ramadan we adjust the weekly GLP-1 injection day (commonly Friday evening or post-iftar) and discuss meal-pattern strategies to keep nausea and hypoglycaemia risk to a minimum. Patients on concurrent sulphonylureas or insulin need additional attention here — we coordinate with your GP where possible.
