The South Asian metabolic-risk panel
The most-requested workup from our Rusholme patients is the metabolic risk panel: HbA1c (per NICE CKS Diabetes, 42–47 mmol/mol is prediabetic, ≥48 mmol/mol is diagnostic of type 2 diabetes), fasting lipid profile (total cholesterol, LDL, HDL, triglycerides), U&E for renal function, LFTs, vitamin D (commonly low in the UK South Asian population due to combination of dietary patterns, skin pigmentation and reduced UV synthesis), ferritin and FBC. South Asian adults develop type 2 diabetes at an average BMI of around 23–25 kg/m², considerably lower than the broader UK average — part of why NICE CG189 applies a 2.5-point lower BMI threshold for weight-management treatment in this group. The metformin and PPI angle
Rusholme has a high prevalence of long-term metformin use — metformin interferes with calcium-dependent B12 absorption in the terminal ileum and is a recognised cause of B12 deficiency in patients on the drug for 5+ years. Long-term PPI use (omeprazole, lansoprazole) for reflux similarly reduces gastric acid and therefore B12 release from food proteins. We screen serum B12 (with active B12 reflex if borderline), folate, MMA and homocysteine per British Society for Haematology guidelines, and if low we can move straight to our Rusholme B12 injections page for loading-dose hydroxocobalamin.
