A mosquito-borne virus that can become severe quickly

Yellow fever is a viral infection spread by infected mosquitoes. It circulates between mosquitoes, monkeys and humans, mainly in parts of Africa and South America. The mosquitoes that spread it can bite during the day, so this is not a disease you only think about after dusk. Many infections begin like a nasty flu: fever, headache, muscle pain, nausea and loss of appetite. Some people start to recover after a few days. A smaller group then becomes seriously unwell, sometimes with jaundice, bleeding, kidney problems, shock and damage to major organs. Severe yellow fever has a high death rate. For travellers, the awkwardness is practical. You may be fit, away for only two weeks, and still be entering a country where yellow fever vaccination is recommended or where proof of vaccination is checked at the border. Previous outbreaks have affected places such as Angola, Brazil, Peru and Bolivia, including cases in unvaccinated travellers.

One injection, but not an automatic yes for everyone

The yellow fever vaccine is a live, weakened vaccine given as a single injection. It is used both to reduce your risk of yellow fever and, where required, to issue an International Certificate of Vaccination or Prophylaxis. For most people who can safely receive it, one dose gives long-lasting protection, and the certificate is generally valid for life once it becomes valid. A booster is only considered for certain travellers who remain at ongoing risk. Timing matters. The certificate is not valid immediately, so booking at least 10 days before you need to enter a yellow fever certificate country is sensible. Earlier is better if your medical history is not straightforward. The vaccine is usually considered from 9 months of age. Babies under 6 months must not receive it, and children between those ages need specialist assessment if travel is unavoidable. Extra care is also needed for people aged 60 or over, pregnant or breastfeeding travellers, people with immune system problems, significant egg allergy, thymus disease or previous thymus removal. Common reactions include headache, muscle aches and a mild fever in the days after vaccination. Serious reactions are rare, but they are the reason the assessment matters.

Countries where yellow fever comes into the conversation

Yellow fever risk is mainly found in tropical and subtropical parts of Africa and Central and South America, plus Trinidad. In Africa, vaccination may be relevant for countries such as Ghana, Nigeria, Uganda, Côte d’Ivoire, Cameroon, Senegal and parts of Kenya or Ethiopia. In South America, it may be considered for parts of Brazil, Peru, Colombia, Bolivia, Ecuador, French Guiana, Guyana and Venezuela. Two separate issues often get mixed together. Second, whether a country requires a yellow fever certificate because you are arriving from, or transiting through, another risk country. Airport transit can matter.
