An infection caught through contaminated food or water

Typhoid fever is caused by Salmonella Typhi, a bacterium passed in human faeces and then spread through unsafe food, water, ice, or poor hand hygiene. Paratyphoid is a similar illness caused by related Salmonella bacteria. Travellers usually pick it up in places where clean water systems and sanitation are unreliable. This is not only a “street food” problem. A drink with ice, salad washed in unsafe water, shellfish, buffet food kept at the wrong temperature, or food handled by someone carrying the bacteria can all be involved. Symptoms can include fever, headache, stomach pain, diarrhoea or constipation, and feeling generally wiped out. Some people become seriously unwell and need hospital assessment. Antibiotics can treat typhoid, but resistance has become a real issue in parts of South Asia and elsewhere. That makes prevention more useful than simply assuming treatment will be easy to find abroad.

What the typhoid vaccine covers, and what it does not

The typhoid vaccine lowers your risk of typhoid fever. It does not cover paratyphoid, and it does not make unsafe food or water harmless. You still need the boring but effective measures: bottled or treated water, hot food served hot, careful hand hygiene, and some judgement around ice, salads and unpeeled fruit. In the UK, typhoid vaccination is commonly given either as a single injection or as an oral capsule course, depending on suitability and availability. The injectable vaccine is generally used from 2 years of age. The oral option is generally used from 5 years of age and is taken as 3 capsules on separate days. Your pharmacist will check which option fits your age, medical history, medicines and travel dates. Try to book at least 2 weeks before departure. Late trips are still worth discussing, but earlier is cleaner. Protection is not lifelong; if you remain at risk, revaccination may be advised after 3 years.
