Chikungunya is a viral infection spread mainly by Aedes mosquitoes. These are not the classic night-biting malaria mosquitoes; they often bite during the day, with busy periods around mid-morning and late afternoon. They breed well around people, especially where water collects in containers near homes, hotels, gardens and building sites. Illness usually starts a few days after a bite. Fever can come on suddenly, often with muscle pain, headache, rash and severe joint pain. Ankles, wrists, hands and other small joints are commonly involved. Most people recover within one to two weeks, but joint pain can drag on for months, and in some cases much longer. Deaths are rare, but chikungunya is not trivial. Babies, older adults and people with underlying medical problems can be more vulnerable to severe illness. For a traveller, the practical problem is simple: there is no specific antiviral treatment, so prevention matters.

What the vaccine can and cannot do

Chikungunya vaccination is designed to reduce the risk of illness from chikungunya virus. It does not replace mosquito bite avoidance, and it will not protect against other Aedes-borne infections such as dengue or Zika. Two chikungunya vaccines are now approved for use in the UK. They are given as a single injection into the muscle. Current UK guidance allows vaccination to be considered for people travelling to areas with active chikungunya outbreaks, long-term or frequent travellers to places with recent chikungunya transmission, and some laboratory workers. Age and medical background matter. One vaccine may be used from age 12 and over. The live vaccine has more restrictions and is currently advised only for certain adults below older-age groups; it may also be unsuitable for people with immune suppression or specific medical histories. A pharmacist or clinician should check this rather than simply booking everyone who asks. Plan early if you can, ideally several weeks before departure. The need for a later booster has not yet been established.
