Loading and maintenance schedules
New B12 deficiency without neurological symptoms: six 1mg hydroxocobalamin injections over 2 weeks, alternate days, per the BNF protocol. With neurological involvement (peripheral neuropathy, glossitis, cognitive change, balance issues): alternate-day injections until improvement plateaus, then 2-monthly maintenance. After loading: pernicious anaemia requires 3-monthly lifelong maintenance; dietary deficiency typically corrects with continued maintenance plus dietary correction or oral cyanocobalamin 1mg daily long-term.

Long-term metformin users (5+ years on the medication) — metformin interferes with calcium-dependent B12 absorption in the terminal ileum. Long-term PPI users (omeprazole, lansoprazole) for reflux. Vegans and strict vegetarians without supplementation. Post-bariatric patients with anatomical malabsorption.
