New B12 deficiency without neurological involvement: six 1mg hydroxocobalamin injections over 2 weeks, alternate days. With neurological symptoms: alternate-day injections until improvement plateaus, then 2-monthly maintenance. Pernicious anaemia: 3-monthly lifelong maintenance. Sale patient profile

Pernicious anaemia patients confirmed by GP who prefer the timing flexibility of a private clinic (NHS dose is free; some patients still come to us for booking convenience). Long-term metformin users (5+ years, with the calcium-dependent terminal ileum absorption issue), long-term PPI users for reflux, vegans and strict vegetarians, post-bariatric patients. We also see a regular cohort of perimenopausal patients in whom fatigue workup reveals coexisting B12, iron and vitamin D deficiency — we run the full panel via our Sale blood tests page rather than treat in isolation.

We can run a serum B12 same-day at our Sale blood tests service — 24–48-hour results from a UKAS-accredited lab. For grey-zone serum B12 (200–300 ng/L) we run MMA and homocysteine, per British Society for Haematology guidelines. We also screen FBC (MCV >100), folate, ferritin, TSH and vitamin D — these often co-exist.
